What is Lip Filler?

Lip filler refers to the injection of hyaluronic acid (HA) gel into and around the lips to add volume, define the lip border, improve symmetry, and enhance the overall shape of the mouth. Hyaluronic acid is a naturally occurring substance in the body, and HA-based fillers are fully reversible using an enzyme called hyaluronidase.

Lip filler is one of the most technically demanding aesthetic treatments to perform well. The lips are highly vascular, highly visible, and subject to constant movement. Poor technique, incorrect product selection, or over-treatment can result in unnatural results, lumps, asymmetry, or in serious cases, vascular complications.

Clinical note

Dermal fillers including lip fillers are prescription-only medicines in England. They must be prescribed by a registered prescriber (doctor, dentist, pharmacist prescriber, or nurse prescriber) following a face-to-face consultation. The 2021 Act makes it illegal to administer fillers to anyone under 18 for cosmetic purposes.

  • Volume enhancement: Adding fullness to thin or flat lips.
  • Definition: Sharpening the vermilion border (the lip edge) for a more defined shape.
  • Symmetry correction: Balancing uneven upper and lower lips.
  • Cupid's bow definition: Accentuating the natural cupid's bow shape.
  • Lip flip: A small amount of filler in the upper lip to create a subtle eversion without adding significant volume.
  • Perioral lines: Softening fine lines around the mouth (often combined with anti-wrinkle injections).
  • Hydration: Improving lip texture and hydration with a very soft HA product.

What to expect — the process

  • Consultation: A thorough consultation should assess your lip anatomy, facial proportions, and desired outcome. A good practitioner will discuss realistic expectations and advise against over-treatment.
  • Numbing: Topical anaesthetic cream is applied for 20–30 minutes before treatment. Many lip filler products also contain lidocaine to improve comfort during injection.
  • The treatment: Filler is injected using a fine needle or cannula. A typical session takes 20–40 minutes.
  • Swelling: Significant swelling is normal for the first 24–72 hours. Final results are visible once swelling has fully resolved, usually within 1–2 weeks.
  • Duration: Results typically last 6–12 months, though this varies by product, volume used, and individual metabolism.
Important safety information

Vascular occlusion — where filler inadvertently enters or compresses a blood vessel — is the most serious complication of lip filler. It can cause tissue necrosis if not treated immediately with hyaluronidase. Always ensure your practitioner carries hyaluronidase and knows how to use it. Avoid practitioners who cannot demonstrate a clear protocol for managing vascular emergencies.
