What is HIFU?

High-Intensity Focused Ultrasound (HIFU) is a non-invasive skin tightening and lifting treatment that uses focused ultrasound energy to target the deep structural layers of the skin — specifically the SMAS (Superficial Musculo-Aponeurotic System) layer, the same layer addressed during a surgical facelift. By delivering precise thermal energy at depths of 1.5mm, 3mm, and 4.5mm, HIFU triggers a powerful wound-healing response that stimulates new collagen production and causes existing tissue to contract and tighten.

The most well-known HIFU device is Ultherapy (FDA-cleared), though many other devices are available including Ultraformer III, SMAS HIFU, and various Korean-manufactured systems. Device quality varies significantly, and the skill of the operator is critical to achieving safe, effective results.

Clinical note

HIFU is the only non-surgical treatment with clinical evidence for lifting the SMAS layer — the same layer targeted in a surgical facelift. This makes it uniquely effective for addressing true skin laxity and early jowling, rather than simply improving surface texture or hydration.

What can be treated?

  • Brow lift: Lifting a heavy or drooping brow.
  • Jowls and jawline: Tightening loose skin along the jawline and reducing early jowling.
  • Neck and submental area: Tightening loose neck skin and reducing a double chin.
  • Décolletage: Improving crepey skin on the chest.
  • Overall facial lifting: A full-face HIFU treatment can provide a subtle but measurable lift across the entire face.

What to expect — the process

  • Preparation: Topical numbing cream is applied for 30–45 minutes before treatment. Some practitioners offer oral pain relief or inhaled analgesia (Entonox) for comfort.
  • The treatment: A handheld device is passed over the treatment area, delivering focused ultrasound energy at precise depths. You will feel a deep, intense heat or stinging sensation with each pulse. A full face and neck treatment takes 60–90 minutes.
  • Downtime: Most patients experience mild redness and slight swelling for 24–48 hours. Some patients report temporary numbness or tingling in the treated area, which typically resolves within a few weeks.
  • Results: Initial tightening may be visible within a few weeks, but the full collagen remodelling effect develops over 2–3 months. Results typically last 12–18 months.
Important safety information

HIFU should not be performed over areas with active skin infections, open wounds, or implanted metal devices. Rare complications include temporary nerve damage, burns, and prolonged numbness. The risk of complications is significantly higher with poorly calibrated devices or undertrained operators. Always ensure your practitioner uses a validated, CE-marked device and has specific training in HIFU.

How to choose a clinic

Ask specifically which HIFU device the clinic uses and whether it is CE-marked and regularly calibrated. Ensure your practitioner has specific training in HIFU and understands the anatomy of the facial layers. Be wary of very low prices — HIFU requires a significant capital investment in equipment, and very cheap treatments may indicate an inferior device or undertrained operator.
